This Consumer Health Data Privacy Policy describes how Better Options Adult Family Home LLC (“Better Options,” “we,” “us,” or “our”) collects, uses, and shares consumer health data under the Washington My Health My Data Act.

This policy applies to consumer health data that is not otherwise exempt from that law.

Consumer Health Data We Collect

Our public website forms are intended for ordinary inquiries. We ask visitors not to submit medical records, detailed diagnoses, medication lists, insurance information, or other sensitive health information through these forms.

However, an inquiry may reveal or allow an inference that an individual is seeking ventilator care, respiratory support, long-term care, or related health services.

Do not send identification documents or additional medical information unless we specifically request information reasonably necessary to verify your identity.

You do not need to create an online account to submit a request.

Verification

We may take commercially reasonable steps to verify your identity and confirm that the consumer health data relates to you.

If we cannot authenticate a request, we may ask for additional information reasonably necessary to complete verification. We will use verification information only for that purpose.

An authorized representative may submit a request when permitted by law. We may request proof of that person’s authority.

Response Time

We will respond without undue delay and generally within 45 days after receiving a request.

When reasonably necessary, we may extend the response period once by up to an additional 45 days. If we require an extension, we will notify you during the initial response period and explain the reason.

We will generally process qualifying requests free of charge up to twice per year. We may charge a reasonable fee or decline to act on requests that are manifestly unfounded, excessive, or repetitive, as permitted by law.

Deletion Requests

When required, we will delete eligible consumer health data from our active records and notify relevant processors, contractors, affiliates, and third parties of the deletion request.

Deletion from archived or backup systems may be delayed until those systems are restored or accessed. Any such delay will not exceed the period permitted by applicable law.

Certain information may be exempt from deletion or retained when legally permitted or required.

Withdrawing Consent

You may withdraw consent for future collection or sharing of consumer health data by using the request method described above.

Withdrawal will not affect processing that occurred before we received and completed the request. It may limit our ability to respond to or provide a service you requested.

Explain why you believe the decision should be reconsidered.

We will respond to an appeal within 45 days and provide a written explanation. If we deny the appeal, you may submit a complaint to the Washington State Attorney General.
